Unable to access my site any more!

I am having fits trying to access my wordpress sliding door theme site. Long story short, my website host mistakenly deleted my account and took off my site. Once the problem was worked out, they restored it. Ever since then, my wordpress site has not been accessible. They tried to repair it from their end and said they’d done all they could and to contact the theme owner. I cannot even log on to my wordpress dashboard and have tried working through the back side of my site. The attached screenshot is the error message that comes up when accessing my page. Something about the sliding door functions on line 217. How can I fix this?
